Claire is a nurse on a general medical ward, and after her third night in a row she stands by the lift at six in the morning and needs a second to work out whether it is Thursday or Friday. This is not the tiredness that follows a hard twelve-hour day. It is a different animal: the body is drained and oddly wired at once, and when she finally lies down at nine she is asleep within five minutes and wide awake at noon for no reason she can name.
The gap between "I am worn out" and "I am out of sync" is more than a figure of speech. A night shift does not simply bolt a few extra hours onto an ordinary day. It sets you against a system that is, at that exact hour, doing the opposite of what you need from it.
Why a night shift is not just a long day
In your hypothalamus sits a small cluster of cells called the suprachiasmatic nucleus. It acts as the body's master clock and takes its cues mainly from light falling on the retina. It sets the hour at which your core temperature drops and at which melatonin, the hormone your body produces in darkness, begins to appear, and the hour at which cortisol climbs towards morning to get you moving.
The clock has no idea you are on duty. Whatever you are doing, between three and five in the morning your body temperature sits at its daily low, and your ability to concentrate and react quickly goes down with it. Chronobiologists call this stretch the circadian nadir. It is the biological floor of performance, and no amount of willpower or coffee argues it away. It can only be softened.
Then comes the harder part. Daytime sleep after a night shift is shorter and shallower than night sleep of the same length. Most people blame the daylight behind the curtains, the noise from the stairwell and a phone that will not leave them alone. Yet even in a perfectly dark, perfectly quiet room, sleep after a night shift usually ends after four or five hours. The Swedish sleep researcher Torbjörn Åkerstedt has documented this for decades: daytime sleep after nights tends to run one to four hours short, because the internal clock cuts it off, sending a loud "up you get" signal through the late morning. It is not the neighbour with the drill. It is your own biology.
What we actually know about shift work
This calls for careful wording, because writing about night shifts slides easily into alarming headlines. Research has long linked shift work with a higher load on the body, mostly through chronic sleep debt and a scrambled rhythm of eating and sleeping. These are statistical associations across large groups, not a verdict on any individual. Plenty of people work rotating shifts for years and do well on them, and the differences between one person and the next are enormous.
The firmest data concern attention and accidents. Simon Folkard and Philip Tucker pulled together dozens of studies from industry and transport in 2003 and arrived at a few consistent findings. The risk of an error or an injury is roughly thirty percent higher on a night shift than on a morning one. It also climbs with every further night in a row, so the fourth night is riskier than the first. The strongest effect, though, belongs to shift length: around the twelfth hour worked, the risk is about double what it was across the first eight.
Out of that comes a practical conclusion that has nothing to do with how tough you are. Overtime glued onto a night shift is the riskiest combination you can put in your own diary. When you get to choose where the extra hours go, put them next to a morning shift.
Sleep after nights: how to lose as little of it as possible
The goal is not eight unbroken hours after a night shift. Most people will not get them, and chasing them mostly produces someone lying in bed, furious, counting what is left. The realistic goal sounds different: take everything the clock will let you have.
The basics stay the same as for ordinary sleep, only stricter. A blackout blind or curtain, and where that is impossible, a proper eye mask. Light landing on closed eyelids still suppresses melatonin, so "nearly dark" does not do the job. Add earplugs or a quiet fan as a cushion of sound, and a bedroom around eighteen degrees, because the body warms up during the day and falls asleep more easily in the cool.
The phone belongs outside the bedroom, ideally in another room altogether. If you have to stay reachable for children or for work, set an exception for a handful of specific numbers and silence everything else. You will notice the difference between a phone on the pillow and a phone in the hallway within a week.
The two-block strategy
Daytime sleep can be split, and for many shift workers that is the best move available. Instead of one block that will not hold anyway, plan two. The common version: a main block straight after the shift, say seven till noon, then a shorter top-up of around ninety minutes in the afternoon or early evening before the next night. The second version suits people who cannot drop off after a shift however hard they try. They take a short block in the morning, deal with life during the day, and move their main sleep to the afternoon, three till seven.
Split sleep sounds like a fallback, but most people bank more hours that way than from a grim vigil in bed between nine and five. The second block carries a bonus: it ends shortly before the shift, so you clock on relatively fresh rather than eight hours after waking.
The anchor that holds the rhythm
The British chronobiologists David Minors and Jim Waterhouse described the principle of anchor sleep back in the early 1980s. If someone on an irregular schedule keeps at least a four-hour block of sleep at the same time every day, the internal rhythms stay steadier even while the rest of the timetable jumps around.
The usual mistake looks like this. On days off you swing back to a completely normal daytime routine so you can share your family's life, and on Sunday evening you clock on for nights with a clock that has spun back the other way in the meantime. An anchor means a compromise. If you sleep from seven to noon after nights, set the alarm on your days off for eleven and go to bed later than feels natural. You lose part of the morning and gain a body that knows where it stands.
Light is the lever that moves the clock
Light is the strongest signal the internal clock receives. That sounds like a truism until you follow it through: you are holding the remote control, and most of the time you are pointing it at yourself.
On shift, you want brightness. Strong light through the first half of the night raises alertness and pushes the clock towards a later schedule, exactly what you want when a run of nights lies ahead. If you work somewhere dim, and many night operations are, it is worth having a bright lamp of your own for the first few hours.
Towards the end of the shift, flip it. Morning light on the way home pushes the clock the other way, towards earlier waking. So sunglasses go on for the car or the tram, and not once you turn the corner but the moment you step out of the building. At home, draw the curtains before you sit down to eat, not when you are already heading for bed.
Now the less intuitive part. Resetting the clock fully to a night schedule only makes sense if you work long blocks of nights back to back. With one or two nights a week and a daytime life in between, a full flip costs more than it pays, because you would be out of sync permanently. For occasional nights, stay anchored in your daytime routine and get through on sleep banked in advance plus a short nap on the break.
Caffeine, food and a nap that earns its keep
Caffeine has a half-life of roughly five to six hours. A coffee at four in the morning is therefore still half at work at ten, which is precisely when you are trying to fall asleep. The working rule: caffeine at the start and through the first half of the shift, the last dose no later than six hours before you plan to sleep. Towards the end of a night, nothing, even though that is the hardest moment of all.
Food follows the same logic. Digestion runs on a daily rhythm too, and at night the body handles a big meal worse than it does at midday, partly because it processes sugars less well after dark. A heavy meal at two in the morning shows up as heavy eyelids an hour later. Smaller portions spread across the shift work better, with enough protein and something warm in there, because warm food at night helps the head as much as the stomach.
That leaves the nap, among the most effective things a shift worker can do for themselves. Sleep before a night shift, ideally in the afternoon, means you walk into the night with a smaller debt. Shift rotation and your chronotype
Not every rota is equally punishing. Charles Czeisler and colleagues published a study from a chemical plant in Science in 1982, where workers rotated backwards: nights, afternoons, mornings. When the rotation was turned forwards (mornings, afternoons, nights) and the interval between changes lengthened, satisfaction with the schedule improved and staff turnover fell. The reason is simple: internal clocks stretch more easily than they shrink. Pushing your bedtime two hours later costs almost nothing, pulling it two hours earlier is usually a fight.
The second thing that decides is your chronotype. Owls, people whose clock naturally sits later in the evening, generally tolerate night shifts better, because at two in the morning their body is still relatively awake. Larks have the mirror-image advantage: a six o'clock start is nearly comfortable for them, while nights are their hardest option. Doves in the middle of the scale handle both averagely and are thrown less by any one shift than by chaos in the rota.

That this is more than an impression was shown in 2013 by the team of Mariana Juda and Till Roenneberg on a large sample of shift workers: chronotype predicted how long and how well someone slept after each type of shift. Evening types got more sleep after nights, while morning types did better around morning shifts.
A second axis runs across all of this, and people differ on it too: steady versus flexible rhythm. Shift one person's schedule by three hours and it barely registers, and the next day they function normally. Shift another person's by the same three hours and they are scattered for a week, even though they slept the same number of hours. Two people with the same chronotype can therefore tolerate an identical rota in completely different ways, which is also why universal advice about shift work so often falls flat. Which shift wrecks you personally? Not the one nobody on the ward wants, but the one after which you specifically are no use the following day. The answer to that question is worth more at the rota negotiation than any general advice, because that is exactly the thing you can realistically swap with colleagues. And if you have no idea where you sit on the axis, a short chronotype test gives you a rough answer in a few minutes.
One more thing, plainly. If sleep problems persist on your days off as well, when no rota is pressing on you, or if you catch yourself nodding off at the wheel on the way home, that belongs with your GP, not in an article about organising shifts.
Claire did not give up nights in the end. She changed two things: she stopped trying to sleep eight hours straight after a shift, and split her sleep into a block from seven to noon plus ninety minutes around five in the afternoon. And she put a pair of sunglasses in her coat pocket, which she now puts on in the hospital doorway rather than in the car park. It sounded like a small thing until, two weeks later, she noticed that after her third night she knew what day it was.
